Falsified welfare checks led to deaths of vulnerable inmates, inquest told
A coroner is to refer all five self-inflicted deaths within nine months at a prison in Nottinghamshire to police, for investigation into possible misconduct in public office.
A jury found multiple failures by private contractors Sodexo led to a vulnerable man taking his own life after spending 53 days in segregation.
At least three prison officers at Lowdham Grange who had a duty to monitor Matthew Osborne three times an hour, failed to do the necessary checks, and faked entries.
Sodexo, which runs the prison, say they accept the findings in full and sent sincere apologies to Matthew Osbourne’s family.
Nottinghamshire Healthcare have also apologised.
While the Ministry of Justice say safety measures at the jail have been strengthened along with “better support for prisoners at risk of self harm”.
